Our innovative Alzheimers/Dementia Mobility Program is growing and changing the lives of patients and their families who, up until our program was developed, didn't have a realistic way to combat the progressive nature of Alzheimers and Dementia and had to accept the inevitable decline that comes as a part of the disease process, that is no longer the case. We need compassionate, motivated nurses to help us expand this program so that we can improve the quality of life for these patients and their caregivers. *Description/Purpose of Position: Registered Nurse- Home Health Case Manager*
The Home Health Registered Nurse acts as the patient Case Manager and provides professional nursing services in support of Foundation Home Health within applicable health care standards, governmental laws and regulations and The AZMO Group Credo.
The RN provides assessment, admissions, care planning and treatment to home health patients. Performs skilled nursing services; including, but not limited to patient teaching and training, observation and assessment and direct care and treatments. Supervises patient care provided by LPN's and aides. Case Manager is responsible and accountable for providing and coordinating patient care across all disciplines. Required to demonstrate the ability to assess patient’s specific needs related to their unique medical condition and social and financial factors and provide appropriate care according to the agencies policies and procedures.
Promotes collaborative practice with other disciplines, physicians, and exhibits the ability to work with others. Performs other duties as required or assigned. Positions at this level are responsible for a broad range of duties. The employee uses professional judgment in selecting appropriate guidelines and in applying general policies and procedures. Significant deviations from established work processes require prior approval. The employee applies knowledge to diversified subject matter, requiring good judgment in applying established procedures and guidelines.

*Education:* Minimum of Associate Degree in Nursing
*Experience:* Minimum one year working as a RN. Previous RN experience in Home Health or Hospice preferred but not required.
*Technical Skills:* Must have strong interpersonal, oral and written communication skills. Effective human relations skills are required for interfacing with team members, all levels of staff, physicians, patients, families and other contacts. Must possess the ability to effectively function in a stressful environment. As required of position, must be able to use computer, telephone, calculator and adapt to new technology as it becomes available and necessary for the job. Must possess critical thinking and problem solving skills. Must be able to operate position specific equipment (BP cuff, manipulate/read syringe, glucose monitor, IV pump, etc.) Must have a positive attitude and even temperament in stressful/conflict situations.
*License/Certification:* Valid Arizona or compact states RN license. Requires BLS within 30 days, Arizona Fingerprint Clearance (or application filed within 20 days of hire - or fingerprints on file with AzBON). Must possess valid drivers license and proof of vehicle insurance.

Pay: $67,560.00 - $85,000.00 per year
